PLEASE BE ADVISED OF THE FOLLOWING DISCLOSURE PROVIDED FOR YOUR PROTECTION:

Laser Hair Removal and Skin Tightening: Please make sure your skin is at its natural color. NO sun exposure, tanning lotions, tanning beds, tanning sprays, and tanning solutions 2-4 weeks before and after your treatment. You must cover all treated areas and wear sunblock. NO Antibiotics, Iron Pills, Retin-A, Aspirin or any blood thinning medications 2 weeks prior to your treatment. NO Accutane or Immunosuppressants in the last 6 months. If you have taken (or are currently taking) these, or any other medications, notify the office immediately. Treatment area(s) must be free of makeup, lotions, deodorant and ointments. Shave 24-48 hours prior to your appointment. AVOID waxing, bleaching, tweezing, threading, Nair, etc. for 4 weeks prior to your appointment. NO facial treatments, microdermabrasion, or chemical peels for at least 2 weeks before and after your laser treatment. NO exercise or deodorant for 24-hours post treatment. NO jacuzzi, sauna, steam room, or hot showers for 24-72 hours post treatment.

Dermal Fillers & Injections: If you’re under 18, Fillers are not recommended. IT IS NOT RECOMMENDED TO RECEIVE FILLERS if you are pregnant, breastfeeding or trying to become pregnant. We will not treat you if you are pregnant, nursing or trying to become pregnant. If you have an active infection in the treatment site, please wait for the infection to subside and be fully resolved prior to your treatment. Make sure you do not have infectious skin problems (acne, etc.). Make sure you don’t have a past history of autoimmune disease. Make sure you do not have any known hypersensitivity to hyaluronic acid. Make sure you do not have any tendencies to develop hypertrophic scarring. Make sure you have not had any radiation therapy, laser therapy, chemical peels or microdermabrasion for 2 weeks before and after treatment. Make sure you do not have immunotherapy treatments, such as Prednisone. Immunocompromised conditions such as (diabetes, HIV, smoking) or bleeding disorders may increase the risks of side effects. The risk of bleeding may be increased with certain medications such as Aspirin, Coumadin, Plavix, some Vitamins such as Vitamin E, Ibuprofen, antibiotics, some herbs and other non-steroidal anti-inflammatories. NO antibiotics at least 2 weeks prior to your appointment. You may not be taking blood thinners or alcohol a minimum of 2 days prior to your appointment. The use of dermal fillers should be postponed in the case of certain inflammatory skin disorders, such as skin sores, rashes, cysts, hives or infection at the injection sites. Please inform us if you have any history or presence of severe multiple allergies, or allergy to any gram-positive bacterial proteins. Do not plan to have surgery on treatment sites within the next 3-4 weeks. Please remove all make up prior to your treatment.

BOTOX: If you’re under 18 BOTOX is not recommended. DO NOT USE BOTOX if you are pregnant or breastfeeding, are allergic to any of its ingredients, or suffer from any neurological disorders. Please inform your provider if you have any questions about this prior to the treatment. WITHIN 2 WEEKS OF YOUR TREATMENT date, you may not have used or taken any Antibiotics. Avoid alcoholic drinks at least 24-hours before treatment (alcohol may thin the blood increasing risk of bruising). Avoid taking certain medications 2 weeks before treatment (e.g. Antibiotics, Plavix, Coumadin, Warfarin, Anti-Inflammatories, Garlic, Vitamin E, Omega-3 capsules, Gingko Biloba, St. John’s Wort, Herbs, blood thinning medications such as Aspirin, Motrin, Ibuprofen and Aleve). Please notify us if you have had surgery on your face or any other changes in the way your face normally looks. Do not plan to have surgery on treatment sites within the next 3-4 weeks. You may not receive chemical peels and radiation therapy for 2 weeks before and after treatment. If you have an active infection in the treatment site, please wait for the infection to subside and be fully resolved prior to your treatment. 3 DAYS BEFORE TREATMENT AVOID topical products such as Tretinoin (Retin-A), Retinols, Retinoids, Glycolic Acid, Alpha Hydroxy Acid, or other “anti-aging” products prior to your treatment. Also AVOID waxing, tweezing, bleaching, or the use of hair removal cream on the area to be treated. Please make sure you do not have hypersensitivity to any botulinum toxin preparation or to any of the components in the formulation. Make sure you are not allergic to cow’s milk, because BOTOX contains trace amounts of cow’s milk protein. Please make sure you do not have any Neurological Disease, such as Eaton-Lambert Syndrome, Lou Gehrig’s Disease/ALS, or Myasthenia Gravis. Please make sure you do not have any swallowing problems, bleeding problems, diabetes, or any heart disease. Please make sure you are not taking any kind of aminoglycoside antibiotics or other drugs known to interfere with neuromuscular transmission. Inform your provider if you have a history of Perioral Herpes to receive advice on antiviral therapy prior to treatment. Please remove all make up, deodorants, sprays, perfumes, and all products prior to your treatment. Please arrive to your treatment clean and bare of all products.

KYBELLA: Please keep in mind that you may not wish to schedule your appointment prior to any special events that might take place within 2-3 weeks of the treatment due to extreme swelling and bruising that may occur post-treatment. It is recommended to discontinue the use of alcohol, blood thinning medication, Aspirin, Motrin, Gingko Biloba, Garlic, Flax Oil, Cod Liver Oil, Vitamin A, Vitamin E or any other essential fatty acids at least 3 days before and after treatment to minimize bruising or bleeding. NO antibiotics at least 2 weeks prior to your appointment. Please consult with your primary Physician prior to discontinuing any medications. If you develop an infection or rash in the area to be treated prior to your appointment, we recommend that you please reschedule your appointment until it resolves. We will not perform KYBELLA treatments on clients with a current or past history of Dysphagia, “trouble swallowing”, currently pregnant or breastfeeding, with the presence of infection at or near the injection sites or under the age of 18. We ask that you arrive to your appointment free of all skin care products or makeup on the treatment area.

If any changes in your medical history occur, it is your responsibility to inform the healthcare professional treating you prior to beginning your treatment.

Policies For All Treatments: You may NOT have any type of laser or injection treatment if you are (or think you may be) pregnant or nursing. Please notify us if you have any medical conditions or history such as: Heart Disease, Thyroid, Diabetes, Cancer, Skin Disease, or anything else that we may need to be aware of. If you are under the age of 18, you must arrive to your appointment with a parent or legal guardian. The practice of medicine is not an exact science and no guarantees can be made concerning the results of such procedures; therefore, there will be NO refunds for any treatments received.

 

Please be advised of our 24-hour Cancellation/No-Show policy; if your appointment is cancelled less than 24-hours prior to your appointment, 50% of your booked services will be auto-charged to the pre-authorized card provided in your file. Please note all pre-paid packages are non-refundable, non-transferable, and may only be used in the office which they were purchased in. All deposits are non-refundable, non-transferable and may not be transferred between laser and injectable treatments.
